Adolescent summer care arrangements and risk for obesity the following school year

Abstract:
This longitudinal study identified common summer care arrangements for adolescents and examined whether those arrangements predicted risk for obesity (Body Mass Index (BMI) ≥ 85th percentile for age and gender) the following school year. Participants were a nationally representative sample of 1766 adolescents ages 10-18 from the Panel Study of Income Dynamics-Child Development Supplement. Results showed that, beyond measures of BMI taken before the summer and several demographic aspects known to predict obesity, youth whose summer arrangements involved regular participation in organized activities (e.g., sports) showed significantly lower risk for obesity than other youth. This was most evident during early adolescence. Youth whose regular summer arrangement was predominated by parent care without organized activity participation showed the greatest risk for obesity.

Nudge to nobesity I: Minor changes in accessibility decrease food intake

Abstract:
Very small but cumulated decreases in food intake may be sufficient to erase obesity over a period of years. We examine the effect of slight changes in the accessibility of different foods in a pay-by-weight-of-food salad bar in a cafeteria serving adults for the lunch period. Making a food slightly more difficult to reach (by varying its proximity by about 10 inches) or changing the serving utensil (spoon or tongs) modestly but reliably reduces intake, in the range of 8-16%. Given this effect, it is possible that making calorie-dense foods less accessible and low-calorie foods more accessible over an extended period of time would result in significant weight loss.

Abstract:
In two studies, one a lab study and the other a real-world study, we examine the effect of manipulating the position of different foods on a restaurant menu. Items placed at the beginning or the end of the list of their category options were up to twice as popular as when they were placed in the center of the list. Given this effect, placing healthier menu items at the top or bottom of item lists and less healthy ones in their center (e.g., sugared drinks vs. calorie-free drinks) should result in some increase in favor of healthier food choices.

The Influence of Bite Size on Quantity of Food Consumed: A Field Study

While research has extensively investigated how portion sizes can influence the quantity of food consumed, relatively little work has been done to explore how bite size influences overall consumption. This research seeks to address this concern. In a field study, we collected data in a restaurant and manipulated bite size by providing diners with small or large forks. We found that diners consumed more from smaller rather than larger forks. Utilizing motivation literature, which ties into the unique factors present in a restaurant consumption setting (e.g., diners have a well-defined goal of hunger satiation because they invest effort by visiting a specific restaurant, choose from a menu, and pay money for the meal), we present our rationale for the pattern of results. Moreover, in a controlled lab study we demonstrate that when these factors are absent, the pattern of results is reversed.

Consideration of the BDNF gene in relation to two phenotypes: Hoarding and obesity

Abstract:
The gene coding for the brain derived neurotrophic factor (BDNF) has emerged as an interesting candidate for multiple brain and brain disorder-related phenomena. The primary aim of the present investigation was to consider the relationship between the BDNF Val66Met variant and two phenotypes: compulsive hoarding as a symptom dimension of obsessive-compulsive disorder (OCD), and body mass index (BMI). We examined the BDNF gene in a large (N = 301) clinical sample of probands with OCD. Participants were classified as hoarding or nonhoarding using a strict, multimeasure grouping approach. Results revealed that the Val/Val genotype was linked with hoarding classification and more severe hoarding behaviors, as well as greater BMI levels. Hoarding status was also associated with greater BMI scores, with individuals in the hoarding group being far more likely to be classified as obese compared with the nonhoarding group. Our findings may provide a distinct avenue through which hoarding and BMI could be linked. These findings are suggestive of a complex gene, body weight, and psychopathology relationship wherein a primitive, survival "thrifty gene" strategy may be conserved and represented in a subgroup of humans manifesting severe hoarding symptoms.

The 2nd to 4th digit ratio (2D:4D) and eating disorder diagnosis in women

Abstract:
Eating disorders are more common in females than in males and are believed to be caused, in part, by biological and hormonal factors. Digit ratio or 2D:4D (the ratio of the 2nd to the 4th digit) is considered to be a proxy for prenatal testosterone (PT) and prenatal oestrogen (PE) exposure. However, how 2D:4D may be related to type of eating pathology is unknown. The relationship between 2D:4D and eating disorder diagnosis was investigated in recovered and currently eating disordered (n = 31) and control (n = 99) women. Mean 2D:4D ratio was significantly lower (indicating higher levels of PT and lower levels of PE) in anorexic (AN) compared to bulimic (BN) women, with controls intermediary. In eating disordered women, 2D:4D was positively and significantly related to current weight, lowest weight and current BMI, with strongest associations for right 2D:4D. Among women, low 2D:4D is related to AN and high 2D:4D to BN, suggesting a differential causal influence of prenatal sex hormones on later eating pathology.

Abstract:
Informed by theory and research on attributions and narrative persuasion, we compared the effectiveness of narrative and nonnarrative messages in changing attributions of responsibility for causes and solutions related to obesity in the United States. We randomly assigned 500 adults to view one of three messages (narrative, evidence, and a hybrid of the two) emphasizing environmental causes of obesity, or a no-exposure control condition. The narrative condition increased the belief that societal actors (government, employers) are responsible for addressing obesity, but only among liberals. This pattern of results was partially explained by the success of the narrative condition in reducing reactive counterarguing, relative to the evidence condition, among liberals. We conclude with theoretical and practical implications.

Children's use of on-package nutritional claim information

Abstract:
Surprisingly little is known about how children respond to marketer-provided information in retail environments and how such information might impact decisions. In this paper, we investigated children's responses to a particular type of marketer-provided information - on-package nutritional claim information. Using a 4 × 2 between-subjects experiment, we examined whether 8- to 12-year-old children are aware of this information, whether it influences their choices, and whether this influence is moderated by the type of claim and/or exposure to an in-school health curriculum. Our findings reveal that this information impacts choices in an intriguing way - it has a backlash effect, causing children to make unhealthier choices when on-package claims are present in the choice set. This negative effect is most pronounced for general claims. Further, the negative impact of on-package claims is reduced, but not eliminated, by health education. Implications for designing more effective ways to communicate nutritional information are discussed.

Weight-Related Behavior among Adolescents: The Role of Peer Effects

Purpose: To investigate whether social interactions in friendship networks influence the following weight-related behaviors of adolescents: exercising regularly, playing an active sport, hours of TV/Video viewing, sleeping six or fewer hours, eating breakfast on weekdays, frequency of eating at fast food restaurants, eating five servings of fruits/vegetables daily, and consuming calorie-dense snacks.
Method: Data from a nationally representative sample of adolescents are used to examine the association between peer and individual weight-related behaviors. Evidence from multivariate regression analysis controlling for an extensive list of individual- and family-level factors as well as school-level unobserved heterogeneity is obtained.
Results: We find a significant positive association between individuals' and friends' behaviors in terms of sports, exercise and fast food consumption. The estimated associations are robust to controls for individual- and family-level factors, unobserved heterogeneity at the school level and our attempts to account for non-random peer selection.
Conclusions: The social transmission of weight-related behaviors is a viable explanation for the spread of obesity in friendship networks documented in recent research. Traditional weight reduction interventions may be fruitfully complemented with strategies that focus on harnessing peer support to modify behaviors.

Education and Racial-Ethnic Differences in Types of Exercise in the United States

Abstract:
Epidemiological research typically focuses on the intensity, frequency, or duration of physical activity, without consideration of the socially meaningful dimensions of exercise. The authors use data from the 1998 National Health Interview Survey (N = 17,455) and information on participation in 15 exercise behaviors to examine educational differences in exercise among non-Hispanic whites, non-Hispanic blacks, and Mexican Americans. Factor analysis identifies three types of exercise: team sports (e.g., basketball, football), fitness activities (e.g., running, weight lifting), and activities that require the use of specialized facilities (e.g., golf, tennis). Cultural capital and human capital perspectives offer insight into different dimensions of the relationship between education and exercise. Whites disproportionately undertake facility-based exercise, blacks tend toward team and fitness activities, and Mexican Americans gravitate toward team sports. Our findings offer insight into the social stratification of health and can aid the design of public health interventions.

Abstract:
While sedentary leisure-time activities such as reading, going to movies, attending cultural events, going to sporting events, watching TV, listening to music, and socialising with friends would seem to contribute to excess weight, a perspective focusing on socioeconomic status (SES) differences in cultural tastes suggests the opposite, that some sedentary activities are associated with lower rather than higher body weight. This study aims to test theories of cultural distinction by examining relationships between leisure-time activities and body weight. Using 2007 data on 17 nations from the International Social Survey Program (ISSP), the analysis estimates relationships between the body mass index and varied leisure-time activities while controlling for SES, physical activities, and sociodemographic variables. Net of controls for SES and physical activities, participation time in cultural activities is associated with lower rather than higher body weight, particularly in high-income nations. The results suggest that both cultural activities and body weight reflect forms of distinction that separate SES-based lifestyles.

Background: Competing theories attempt to explain changes in total energy (TE) intake; however, a rigorous, comprehensive examination of these explanations has not been undertaken. Our objective was to examine the relative contribution of energy density (ED), portion size (PS), and the number of eating/drinking occasions (EOs) to changes in daily TE.
Methods and Findings: Using cross-sectional nationally representative data from the Nationwide Food Consumption Survey (1977-78), Continuing Survey of Food Intakes of Individuals (1989-91), and National Health and Nutrition Examination Surveys (1994-98 and 2003-06) for adults (aged ≥19 y), we mathematically decompose TE (kcal/d) to understand the relative contributions of each component-PS (grams/EO), ED (kcal/g/EO) and EO(number)-to changes in TE over time. There was an increase in TE intake (+570 kcal/d) and the number of daily EOs (+1.1) between 1977-78 and 2003-06. The average PS increased between 1977-78 and 1994-98, then dropped slightly between 1994-98 and 2003-06, while the average ED remained steady between 1977-78 and 1989-91, then declined slightly between 1989-91 and 1994-98. Estimates from the decomposition statistical models suggest that between 1977-78 and 1989-91, annualized changes in PS contributed nearly 15 kcal/d/y to increases in TE, while changes in EO accounted for just 4 kcal/d/y. Between 1994-98 and 2003-06 changes in EO accounted for 39 kcal/d/y of increase and changes in PS accounted for 1 kcal/d/y of decline in the annualized change in TE.
Conclusions: While all three components have contributed to some extent to 30-y changes in TE, changes in EO and PS have accounted for most of the change. These findings suggest a new focus for efforts to reduce energy imbalances in US adults.

A Multivariate Analysis of Federally Mandated School Wellness Policies on Adolescent Obesity

Purpose: To evaluate the effects of school wellness policies mandated by the 2004 Child Nutrition and WIC Reauthorization Act on the prevalence of overweight and obesity among adolescents.
Methods: Multivariate logistic regressions, adjusted for clustering within school districts, were used to estimate the effects of district-level wellness policies on the odds of overweight and obesity among adolescents. The analyses were performed on a population-based sample obtained from the Utah Population Database, a compilation of vital characteristic, administrative, and genealogical records on all residents in Utah. Models controlled for individual, maternal, and familial characteristics, as well as characteristics of school district of residence. Self-reported body mass index was taken from drivers license data.
Results: Each additional component included in a district's wellness policy was associated with as much as: 3.2% lower odds in the prevalence of adolescent overweight (OR = .968; 95% CI = .941-.997), 2.5% lower odds of obesity (OR = .975; CI = .952-.997), and 3.4% lower odds of severe obesity (OR = .966; CI = .938-.995). Wellness policy components related to diet were significantly associated with lower body mass indexes across all three thresholds, whereas those related to physical activity had significant associations for lower odds of severe obesity only.
Conclusion: Results suggest that school wellness policies can significantly reduce the risk of adolescent obesity. Further research should address specific policy components that are most effective in various populations, as well as the level of commitment that is required at both the school- and district-levels for sustained effect.

Abstract:
Cells, tissues and organisms have the ability to rapidly switch substrate oxidation from carbohydrate to fat in response to changes in nutrient intake, and to changes in energy demands, environmental cues and internal signals. In healthy, metabolically normal individuals, substrate switching occurs rapidly and completely; in other words, substrate switching is ‘flexible'. A growing body of evidence demonstrates that a blunted substrate switching from low- to high-fat oxidation exists in obese individuals, as well as in pre-obese and post-obese, and that this ‘metabolic inflexibility' may be a genetically determined trait. A decreased fat oxidation can lead to a positive energy balance under conditions of high-fat feeding, due to depletion of glycogen stores that stimulates appetite and energy intake through glucostatic and glucogenostatic mechanisms, e.g. hepatic sensing of glycogen stores. Several genetic polymorphisms and single-nucleotide polymorphisms have been identified that are associated with low-fat oxidation rates and metabolic inflexibility, and genetic identification of susceptible individuals may lead to personalized prevention of weight gain using fat oxidation stimulants (‘fat burners') in the future.

Is it Food Quality or Quantity that Responds to Changing Income?

Abstract:
Since the early 1990s, the retail market for leafy green vegetables has been divided between largely unprocessed products and bagged salads. The latter embody kitchen labor and economize on storage costs, in effect offering consumers convenience as a product attribute. Using time-series retail purchase data, this paper shows that changes in income immediately influence how much convenience consumers of leafy greens purchase. There is no statistical evidence that total quantity of leafy greens purchased responds to short-run changes in income. Hence, consumers appear to adjust to income changes by choosing food quality rather than quantity.

Food Insecurity and Obesity in New York City Primary Care Clinics

Background: Hunger continues to be a problem in New York City; paradoxically, the city also has disproportionally high rates of diabetes and obesity. Some research suggests that food insecurity leads to obesity.
Methods: We undertook a cross-sectional "card study" in which doctors working at 8 New York City area primary care practices administered a brief, anonymous survey to patients they saw during clinic sessions. The survey included a 2-question food insecurity screen and questions about enrollment in nutrition assistance programs. Height and weight were also measured at the visit.
Results: Cards were completed for 558 patients (65.1% female; 74.7% adults; 78.5% conducted in English). Fifty-five percent of patients were receiving some form of food assistance. More than half of patients (51.7%) reported some degree of food insecurity. Of adult participants, 21.8% had normal weight, 29.1% were overweight, and 48.2% were obese. Food insecurity was significantly associated with increasing body mass index in women not receiving food assistance. There was no significant association between body mass index and food insecurity in children.
Discussion: In an urban population, overweight and obesity are very common as is food insecurity. We found an association between food insecurity and obesity only among women not receiving food assistance suggesting a possible protective role for food assistance. Providers should consider food insecurity in similar populations when trying to address obesity.

Influence of Licensed Spokescharacters and Health Cues on Children's Ratings of Cereal Taste

Objective: To investigate whether licensed media spokescharacters on food packaging and nutrition cues affect young children's taste assessment of products.
Design: In this experimental study, children viewed 1 of 4 professionally created cereal boxes and tasted a "new" cereal. Manipulations included presence or absence of licensed cartoon spokescharacters on the box and healthy or sugary cereal name.
Setting: Shopping center in a large northeastern city in December 2007.
Participants: Eighty children (mean [SD] age, 5.6 [0.96] years; 53% girls) and their parents or guardians.
Main Exposure: Licensed cartoon characters and nutrition cues in the cereal name.
Outcome Measures: Children rated the cereal's taste on a 5-point smiley face scale (1, really do not like; 5, really like).
Results: Children who saw a popular media character on the box reported liking the cereal more (mean [SD], 4.70 [0.86]) than those who viewed a box with no character on it (4.16 [1.24]). Those who were told the cereal was named Healthy Bits liked the taste more (mean [SD], 4.65 [0.84]) than children who were told it was named Sugar Bits (4.22 [1.27]). Character presence was particularly influential on taste assessments for participants who were told the cereal was named Sugar Bits.
Conclusions: The use of media characters on food packaging affects children's subjective taste assessment. Messages encouraging healthy eating may resonate with young children, but the presence of licensed characters on packaging potentially overrides children's assessments of nutritional merit.

Is Being in School Better? The Impact of School on Children's BMI When Starting Age is Endogenous

In this paper, we investigate the impact of attending school on body weight and obesity using a regression-discontinuity design. As is the case with academic outcomes, school exposure is related to unobserved determinants of weight outcomes because some families choose to have their child start school late (or early). If one does not account for this endogeneity, it appears that an additional year of school exposure results in a greater BMI and a higher probability of being overweight or obese. When we compare the weight outcomes of similar age children with one versus two years of school exposure due to regulations on school starting age, the significant positive effects disappear, and most point estimates become negative, but insignificant. However, additional school exposure appears to improve weight outcomes of children for whom the transition to elementary school represents a more dramatic change in environment (those who spent less time in childcare prior to kindergarten).

Vulnerability to Fat-Stigma in Women's Everyday Relationships

Abstract:
Obesity is understood as a major medical and public health challenge, but the stigma attached to it also creates extraordinary suffering. The pervasiveness of morally negative views toward the overweight and obese, such as laziness and lack of self-control, are undeniable in mainstream U.S. society, situated both institutionally (such as health care barriers or media stereotypes) and interpersonally (such as the negative comments of others). To test basic pathways related to the etiology of women's vulnerability to feeling "fat-stigma" in interpersonal relationships, we present a study conducted between August and November 2009 that combines social network, anthropometric, body image, and interview data for 112 women aged 18-45 years, living in Phoenix, Arizona, U.S., and linked follow-up interviews with 823 of their social ties. Based on the proposition that some social network characteristics should amplify the personal experience of stigma, and others should ameliorate it, we ask: what relationship qualities make women more sensitive to the judgments of others about their weight? We find that what others say about women has only a very limited influence on how women judge others' negative views of their weight once actual body size is taken into account, but that women are more influenced by the opinions of those they are closer to and interact with more often. Ultimately, the degree to which women perceive themselves to be judged by others regarding their weight is not well explained by the actual opinions of people in their networks, either known or unknown to them. The assumption that social network norms exert considerable influence on people's stigma experiences needs to be carefully evaluated, at least in the domain of overweight and obesity.
